The Amputee Coalition is accepting applications for the 2014-2015 Christina Skoski, MD, Scholarship.
The scholarship will provide $1,000 to a full-time undergraduate student with a congenital limb difference or an amputation. The candidate must also be, or have a parent or legal guardian who is, a registered friend of the Amputee Coalition.
Applications should be accompanied by the candidate’s complete, official high school or college transcript, documentation of ACT or SAT scores, a personal statement from the applicant and a letter of recommendation from an academic adviser.
The candidates’ Amputee Coalition record number, which will be reviewed prior to the application, must also be included with the application.
All documents must be mailed to the Amputee Coalition and postmarked on or before the April 7 deadline.
The entire award is made in one payment to the student’s higher education institution upon verification of the student’s enrollment and full-time status. The chosen scholarship recipient will be notified no later than June 9.
